Waiver Form -- 2011/2012
(valid Sep 2011 - Aug 2012)
IMPORTANT NOTES (Please Read):
- All participants must have completed and
submitted a current waiver
form before participating in any TRSL activity.
- Each player is only required to fill this out ONCE during the year from Sept 1, 2011 to Aug 31, 2012. IE. once you fill it out, you're covered
for all TRSL teams you play on until September
2012.
A Note About Privacy of Information:
- Your information will NEVER be sold, traded
or given away to any outside party.
- We will on occasion (usually no more than
3 times a year) send an email alerting you
to the start of the upcoming season, or other
important news.
- You will always be given to option to stop
receiving these messages -- we don't want
to bother you if you are not interested.
